About 20-25% of all cases of infertility are related to tubal factor infertility. Tubal factor infertility may be due to one or more of the following reasons:

Completely blocked fallopian tubes, either one or both, by scar tissue inside the tube or by prior surgery such as tubal ligation
Tubal scarring without blockage of the tube but with damage to the cells that line the inside of the fallopian tube
A disturbance in the architecture of the tube such as with a hydrosalpinx (water-filled tube) or scar tissue outside of the tube which distorts the tube

Pelvic endometriosis
Pelvic infection, such as pelvic inflammatory disease (PID)

There are many anxious moments that a woman experiences during pregnancy. Hearing the first heartbeat, waiting for that first kick and wondering what delivery will be like are among some of the concerns that a woman will feel when she is pregnant. However, there are complications that can arise during pregnancy. One of the earliest complications is tubal pregnancy.

Tubal pregnancy occurs when an embryo implants itself in the wrong location. While a normal fertilized egg will makes its way down the fallopian tube and into the uterus before attaching, a tubal pregnancy will involve a fertilized egg that erroneously attaches itself to the tubal wall or even the cervix. These kinds of pregnancies cannot be successful and can cause rupturing of the tube (provided implantation took place in the fallopian tubes) if not caught in time.

When a woman becomes pregnant, it is important that she visits her doctor early on to be monitored for the possibility of this complication. However, the risk is even higher in women who have had a tubal reversal procedure or any kind of tubal surgery done.

Tubal reversal is a procedure where a woman has had her tubes tied and then reattached later by way of surgery. Because there can be scar tissue and other factors involved, tubal pregnancy risks are higher in these women. In fact, these risks remain whether the woman is having her first or fifth child after a tubal reversal. However, the risk of tubal pregnancy in a woman after tubal reversal is only 10 to 15%.

This is why it is necessary to follow your tubal reversal surgeon’s instructions once you think you might be pregnant. If the doctor you are considering for your reversal surgery does not have any pregnancy instructions to follow, it is highly recommended you continue your search for another tubal ligation reversal doctor.

Following the instructions from the Chapel Hill Tubal Reversal Center, we know it is important for a doctor to regularly check HCG levels in a woman after becoming pregnant when she has had a tubal reversal. Once the doctor has been able to do a vaginal ultrasound and see a sac in the uterus, she will likely be out of the woods when it comes to worrying about tubal pregnancy.

However, if the HCG levels are rising but the doctor cannot see a sac then there is a strong concern that a tubal pregnancy is happening. In these cases, the doctor will recommend using a medication to end the pregnancy before it can progress. Because the embryo will have implanted in the wrong area, there is nowhere for a healthy baby to grow. Plus, it is dangerous to the mother and will most likely result in the loss of a tube.

If the medication is used to terminate the erroneous tubal pregnancy, then you have saved your fallopian tube. This means you have as much chance to get pregnant as you had before the ectopic pregnancy rather than lessening your chances as you would should you lose the fallopian tube through a near rupture or the even more dangerous rupture.

This just shows how important it is to follow a well-defined pregnancy protocol if you think you are pregnant after tubal reversal. Do all you can to catch a tubal pregnancy as soon as possible.

What makes you fit for having a tubal reversal?

Patients need to be more aware of the fertile possibilities of tubal reversal before going for a tubal reversal. The factors that go into whether a tubal reversal is right for you are:

Age: Better if you are less than 40. Analysis of the fertility results demonstrated that age was the most significant predictive factor. This directly links to ovulation. Decide about a tubal reversal at the right time. There is a sharp turn down in fertility that starts to occur approximately at the age of 36 in women. Even so, the chances of pregnancy stay good until women reaches 40. If a woman is beyond the age of 43, the chances of producing eggs in good health can be much lower. Type of ligation: As there are many types and methods of performing bilateral tubal ligation (ie by rings, clips, fulguration, Parkland, Pomeroy, Falope ring, coagulation, and Irving), the damage to the tubes may differ from one patient to another. If the tubal ligation is performed without excessive damage to the tubes, nearly all types of tubal ligations can be reversed with a tubal reversal procedure. To ensure patency, after tubal reversal a colored dye is used to examine the patency of the fallopian tubes. Blockage site on the tube. HysteroSalpigoGram (HSG) is the test that allows Dr. Morice to better evaluate the blockage site on the tubes in the prospective patient for tubal reversal, and to get a better idea of how much proximal tubal length is available for a tubal reversal or reanastomosis. If the ligation method caused excessive scar tissue on the blockage site, this could minimize the potential of fertility later. The amount of healthy tube remaining is extremely important when considering a tubal reversal to regain fertility. Hormone levels: Checking the hormone levels is an excellent indicator of fertility and must be considered for all patients over the age of 30 before a tubal reversal. This includes a day 2-4 FSH level and estradiol level. Normal uterus: All patients undergoing a tubal reversal should have an ultrasound report prior to the procedure. Dr. Morice performs this as part of the routine work-up and this is included in the price of the tubal reversal procedure. Some other factors: There are other factors that provide good predictive value for success with tubal reversal. These include your general health, obstetrical history, partner’s health, social habits such as smoking and drinking, and frequency of intercourse.

Hi Everyone are there fees that are not disclosed with the total cost of TR? Also my doc says I don’t have much of my right tube and I have lots of scar tissue from 2 previous c-section and cyst removal surgery they said I was stupid and wasting $ if i get a TR. I think this is better and less costly then IVF. What do you think?

Hello. Nope no hidden fees. Have you submitted your Operative Report to Dr B for his review? I went thru IVF in 05 without success. I had my TR 7/23/07, if not successful this month, i have the comfort of knowing I can try again with each month. Good Luck!

No hidden fee, everything was just as they said it would be. I wouldn’t change anything about my experience. I had my TR on 06/11/07 and got found out that we are expecting 08/17/07. My doctor also told me that he didn’t think the reversal would work. DR. B makes miracles happen..

No hidden fees- let me tell you what kind of a dr. Dr. berger is—The day before my surgery one of the girls was having her TR done and Dr. Berger seen she had a hernia- he fixed it!!!! Then the next day the girl after me also had a hernia and he fixed hers as well- I know he made one of the C-section scars look MUCH better too…
You could not ask for a better dr. you will just love Dr. Berger adn his staff. From the moment you get there even when you get home – someone will call and check on you… They truly are the best!!!!
Several women on end up with one tube for what ever reason- but you only need one and honestly I would NOT let any other Dr. do my TR- Dr. Berger knows what he is doing and will do the very best for you… With Dr. Berger and his staff you are someone!!!!! Not just another patient or pay check– They really really care!!!!

No hidden fees whatsoever. Matter of fact, my husband accidentally overpaid by $100 when making the appointment, and as soon as we arrived they presented him with a refund check! How’s THAT for honesty?!?! Although Dr. Berger and his staff does this many times over, each and every week, we were truly treated like we were the most important family in the world by EVERYONE. They are amazing, professional people that go that extra mile to make everyone comfortable and satisfied. They are true blessings to this world, and I’m currently 20 1/2 weeks pregnant with my baby boy to prove it! Something I felt was impossible until meeting Dr. Berger and his staff!

Other than your hotel stay and getting there, nope… none. You do have to have your B/W done by your doc, but if you have insurance, it covers all or most of it in the majority of cases.
You can pay a small amount over the surgery fee for a lap first, and if Dr. B cannot repair your tubes (at least one) then you will get a partial refund. But if he can repair at least one tube, he will come out and tell your guest and proceed with the TR. If you’re worried, that is a good “just in case” option.
Above all, you can email or call CH and someone will reply/call you back/talk to you. They are very informative and do all they can, in my experience, to help you with any questions you have. And NO, you won’t get a sales pitch… they’ve been doing this for awhile and understand its a HUGE decision.

What is Vasovasostomy?

The standard vasectomy reversal operation is called a vasovasostomy.

Your surgeon will make a cut in each side of your scrotum, usually in the same position as your vasectomy scars. Alternatively he or she may make one cut in the centre of the scrotum.

The ends of the vas deferens are freed from any scar tissue and carefully pulled through the incisions. Your surgeon will sew the two ends together using dissolvable stitches. He or she will then place them back inside the scrotum and stitch the incisions. The dissolvable stitches will disappear after about a week.

Sometimes it is not possible to rejoin the tubes in both testicles and only one is rejoined. It may still be possible to get your partner pregnant however.

If you have experienced the frustration and disappointment of an unsuccessful reversal procedure, you may want to ask a vasectomy reversal doctor if another attempt is worthwhile.

Generally, vasectomy reversal success rates for a second reversal are slightly lower than for first-time procedures. This is usually because the vas has been obstructed longer and scarring increases each time surgery is performed.

The good news is that an unsuccessful first reversal attempt can sometimes be successful the second time around. The following information can help you understand the reasons why the first reversal was unsuccessful and some factors to consider if you are thinking about a second attempt reversal.

When the First Attempt Fails

Vasectomy reversal success rates are influenced by several things. Factors that include success are:

Second Attempt Vasectomy Reversal Techniques

Generally, if your vasectomy reversal doctor finds the vas deferens have little damage caused by the original vasectomy reversal surgery, the chances of a successful second attempt reversal are greater.

Once the condition of the vas deferens has been determined, the surgeon must choose which of the vasectomy reversal techniques to proceed with.

When the vas deferens appears healthy and the first attempt didn’t produce a lot of scar tissue a vasovasostomy is usually performed. However, if injury to the vas deferens is more extensive, or if scarring or blockage of the epididymis is suspected or detected, vasectomy reversal techniques become more complicated and lengthy. In such cases, vasectomy reversal surgeons may opt for vasoepididymostomy for the second reversal.

This procedure attempts to bypass the obstructed or damaged portion of the vas in order to successfully reconnect the tiny tubes at a higher point near the epididymis. Vasoepididymostomy can result in a successful outcome. It’s important to note that men, who needed a vasoepididymostomy as the first reversal procedure, will not be the best candidates for a second attempt reversal.
