Infertility is the condition wherein a couple is not able to conceive. This can pose a big problem between married couples. The husband can be infertile if there is a problem with his sperm. This can either be lack of sperm or a problem with sperm motility. For the wife, she can have problems with ovulation, obstruction of the fallopian tubes, and hormonal defects. And there can be cases wherein these conditions do not exist between the married couple but they are still unable to conceive.

Because of the growing problem with infertility, many infertility cures were discovered and formulated. Conventional therapies for infertility are surgery and drugs.

Surgery. Performing reproductive surgery for men or women is an infertility cure. Surgery is used in 85% to 90% of infertility cases. For men, a vasectomy reversal and varicocele repair may be performed as infertility cure. For women, it can be removal of fibroids or endometriosis implants.

Drugs. There are drugs used as infertility cure. Some drugs were formulated to stimulate the production of eggs. Examples of these are Serophene, Clomid, Repronex, and Pergonal. Assisted reproductive technology is performed when all other treatments have been used and the couple still cannot conceive. Assisted reproductive technology procedures are in vitro fertilization, intracytoplasmic sperm injection, blastocyst culture, cryopreservation, and artificial hatching.

These are the different known infertility cures. But before a cure is done, certain risk factors for infertility are looked into. Some risk factors are stress, age, smoking, alcohol intake, drug use, and others.

What is Tubal Reversal?

A tubal reversal is a procedure to reconnect the fallopian tubes in a woman who has previously undergone surgical sterilization, known as tubal ligation. In a tubal reversal operation, the surgeon removes the portion of each fallopian tube that has been blocked or cut. Once the damaged portion has been removed, the tubes are reconnected to form a complete, open fallopian tube. The fallopian tubes are the passageway through which the released eggs travel and meet with a man’s sperm and become fertilized.

A tubal reversal, also known as tubal reanastomosis, is usually performed when a woman wants to try to achieve pregnancy after undergoing a tubal ligation. In many cases, surgery for tubal reversal is successful. However, a number of factors can affect the success of the procedure. Estimates vary, but health experts approximate somewhere between 50 to 75 percent of tubal reversals are successful in reopening the fallopian tubes. However, the success rate may be much lower.

A woman considering a tubal reversal must first determine if she is eligible for the procedure. In some cases, the initial sterilization may have prevented any hope of reversal. In addition, reversal may be difficult if sterilization was achieved through cauterization (using electric heat). Minilaparotomy. A small incision (1 to 3 inches long) is made below the navel and the physician raises a portion of each fallopian tube to repair it. Laparoscopy. The patient’s abdomen is inflated with gas (carbon dioxide), which helps the surgeon locate the fallopian tubes. A small incision is made below the navel, and a laparoscope is inserted into the abdomen. Instruments are inserted into the same incision or a separate incision and the tubes are repaired.

Magnification and microsurgical techniques are used to repair the fallopian tubes. The ends of the tubes are cut to expose the inner, open space of the tube, called the lumen. These openings are then fused using a microscope and very fine suture material. The size of these sutures is smaller than the width of a human hair. After the tubes have been reattached, the outer covering (serosa) of tubes is sutured together over the top of the inner tube.

National Infertility Week – What About Infertility Because of a Tubal Ligation?

During National Infertility Week, starting today October 20th, a lot of attention in the media will be given to the treatment of In Vitro Fertilitzation (IVF). This is a technologically advanced treatment for most causes of infertility. But what about those couples who want to have a baby but can’t because of a previous tubal ligation? There are hundreds of thousands of infertile couples in this group in the US. Is IVF the best treatment for them? Let’s compare the two treatments to see which might be the best one for women whose tubes have been tied.

Tubal reversal is a method of rejoining the fallopian tubes where they were cut. The preparation is minimal for the surgery and includes reviewing your tubal ligation operative records. The operation takes about an hour and is done as outpatient surgery at Chapel Hill Tubal Reversal Center. This cuts the cost in half or less than if done as an in-hospital operation.

Now let’s look at IVF in comparison. There are hormonal shots that you have to get in order to stimulate your body’s egg production and also to keep it from going into ovulation prematurely. These can take from one to two weeks or longer and are just the preparation part.

Next will be the removal of your eggs. Of course, the doctor will need to be sure of the right time to do so and will do this by monitoring with hormonal blood tests and ultrasound exams to be sure to catch the eggs at the right time. Now, comes the part you have been taking all those shots for. You will have the fertilized eggs implanted into your body. You will most likely get several fertilized eggs, which actually have begun developing into embryos, placed back into your uterus. You will get more injections at this point as well.

This is when you hold your breath hoping everything will go OK. Of course,this will depend upon things like how skilled your doctor is and how careful he was in implanting the embryos among other things. If it doesn’t work, you may have had some extra embryos frozen.

Before you make a decision to have IVF done, you should probably take a look at the success rates. Most commonly the success rates of any one cycle are given as 20 – 30% that you will be successful. You have a higher likelihood of success with more eggs implanted but don’t want to go overboard as that could lead to some problems as well. You will find that three embryos is about the most any doctor will implant. Still, one in every three pregnancies that occur after IVF is a multiple pregnancy. Babies conceived as a result of IVF are more likely to be born prematurely and have health problems later in life.

Getting back to the option of tubal reversal, the success rates will vary depending upon your chosen surgeon. I am new here, well I have been reading for a while but just registered. I got a tubal (the rings, but not cut and burnt) in November 2005. The reason was that I had had 6 miscarriages. The last was at 3 months. Found out that it was low progesterone. But, my emotions got the best of me and I decided a few days after my last miscarriage to get a tubal. My miscarriage was on Sept 18, 2005 and I was getting my tubal on November 24, 2005. I regret it with every inch of my being. I would love to have one more baby. I have three ages 8,7 and 4. I should also say that I just turned 27 so age is in my favor I imagine.
My questions:
I am in Canada and was wondering why people are choosing to see Dr. Berger instead of getting it done in Canada. I am definitely getting the reversal but I am investigating the best options before I decide where to go.
Cost: For those in Canada, is it not cheaper to get it done here instead of with Dr. Berger? I am just wanting to know why you chose to go to the states as I will get a better idea of where I would like to go.
I just got the clamps (rings) so are there quite a few success stories with getting pregnant after? I know that if they are cut and burnt I am sure that the chances are not as great as the rings.
Also, my specialist that did the tubal told me that the rings would cut off circulation and would eventually make my tubes split apart. Is this true? Or has anyone heard of this?
Ever since my tubal I have had wicked pain at ovulation and right before my monthly. I am sure it is because of the clamps. My regular doc says he thinks I have endometreosis. I am actually going for an ultrasound today to check for that. I am sure that it is just the clamps. I wonder if surgery cost would be covered if the clamps are being a problem. Probably not!
Also, I have been reading about bmi. That is not a problem for me as mine is aroud 30. But, I am curious as to why that matters.

Well I do not live in Canada but the reason we choice to look to Dr.Berger for a TR is because of his studies in to this, his care of every woman that goes to him, and the fact that he is well known and only does TR’s. I did not have the rings but I too have had the pain right before my AF and it is all because of the blood flow being cut off my doctor told me. So getting the TR would restore that. The getting pregnant after a TR if you had the rings I can not tell you. Our bodies are all different so it all depends on what your tubes are like after the TR I would think. Dr.B has things posted about this on his site. The BMI is important when it comes to putting someone to sleep and how much body fat the doctor has to go through to get to the tubes to repair them. This is what my doctor told me. I hope I was at lest a little help. If you want to come in to the waiting room part 42 and jump in and start asking ?’s the lady’s will gladly answer. Hi- I thought i would jump in quickly.

The Chapel Hill Tubal Reversal Center is the only center in the United States which specializes in the reversal of tubal ligation. They don’t do them periodically or on the third Thursday of every fourth month- instead every day, 5 days a week and every month of the year. The center keeps comprehensive statistics and follow-ups on patients for at least 12 months after the surgery so the success numbers are real and verified.

Also people really love to come to Chapel Hill to experience Tar Heel basket ball and to see the birth place of Air – his Highness- Micheal Jordan!

To have any permanent sterilization the tube must be separated. They can be ligated (tied), cut, and then they can be cauterized (fancy medical way of saying burnt)- this causes the tubes to close and separate. The rings and clips achieve the same result but through different mechanisms. These devices put so much pressure on the tube in a small area which causes the blood supply to stop to that area. This causes – avascular necrosis- or more simply, causes the tissue to that area to die and the tube separates and heals itself over. The benefit to the rings and clips is they are both very effective (low rates of failure and ectopic pregnancy) and minimal amounts of tubes are destroyed and reversal is theoretically easier.

Pain after ligation. Rarely do the devices cause pain after ligation. I have occasionally seen a rare patient in which the actual clips were the cause of the pain which resolved when the clips were removed.

It is possible you could have endometriosis but more commonly pain and cramps are due to other conditions such as adenomyosis- or just worsening periods as you get older (I am not calling you old-okay!). Patients after tubal ligation stop hormonal methods which usually make cramps and pain better- this is why some people have the perception that things got wore after the ligation. Some of the patients at the center have post tubal ligation syndrome and seek reversal for this reason.

Tubal Reversal Message Board members discuss testing for pregnancy after outpatient tubal ligation reversal surgery. One tube reversal patient had her menstrual period, but found out a few days later she was actually pregnant. Another sterilization reversal patient had a very faint line two days in a row and confirmed a positive pregnancy after tubes reversed by having a blood test at her doctor’s office. Pregnancy success can happen after tied tubes are untied!

Well ladies, my Af was almost two days lat but showed up on Dec 3rd. I started having CM thick on Friday cd8 so I tested for ovulation came back positive. Sunday still continuing with thick cm went to church was there 30 minutes and began to get hot and sick at my stomach 5 minutes later was n the bathroom throwing up. Threw up twice then left church went home and threw up again. Took some phenergan and slept most of day. Woke up this morning got ready for my HSG test . Took a HPT not sure why just did. Came back positive! went to test told DR she ordered a HCG went home took 3 more HPT all had faint pink lines. Still waiting for test results had DH pee tested ( no faint pink line) So I knew I was pregnant but now need the numbers. Numbers came back @ 40. So if i calculated correct I would be 24 past ovulation! Do these numbers seem low to anyone? I also am cramping so worried!

I wish none of us worried as much as we do, and I find it almost funny that we are the first ones to say “try not to worry” but worried our little heads off. I’ve seen lower numbers and successful tubal reversal pregnancies . I cramped up until my 13th week. I worried until I quit cramping. I hope that makes you feel better and maybe worry a little less. Lets wait and see how your numbers do the 2nd and 3rd draw. Good luck. I will be praying for you

My first number was 44. I am a little confused though .. AF showed on Dec 3? Or do you mean Nov 3? Dec 3 would only make you CD 13 .. and many don’t even ovulate by CD 13. Maybe I am reading it wrong ..

I honestly can say I was one that did not worry. I took a HPT once AF didn’t show up. It was a very faint line (barely visible). Took another the next day with FMU and the same result. I called the doctor and went to have BW. My HCG was 72 then the second draw it was 225. I never had another draw and went in for US at 6 weeks and everything was right where it was supposed to be.

As long as your numbers are doubling it doesn’t matter where it starts out at. If you read the back of the HPT box it will tell you also that you are more likely to get a – result when you are actually pregnant than you are to get a + result when you aren’t. Relax and keep breathing!!!

Congrats to you! I couldn’t be happier if it were me! I mean that. I think going through this tubal reversal journey together makes this so much more special. We had our surgeries the same day and have grown closer and closer since then. Like I told you last night, just be excited!! I know it is hard but this is great news! I love you girl!!

Congrats! I hope that everything works out great for you – however…. You are right, if you really are 24 DPO than the number is low. I wonder if you didn’t ovulate later that you thought? You know a OPK will react as a +++ if it detects hcg in your urine. It happened to me.

AF was a day late and I tested and got a BFN. AF arrived the next day. But my bb’s stayed swollen and AF was very short and light. At around CD 9 I felt right sided and rectal pain. It went away after one day. Then it came back very lightly. I used an OPK on what I thought was CD14 on the new cycle and it was +++. Then 2 days later I started bleeding again this odd purplish blood.

I went in for a blood draw and my level was 120. They did an u/s and found my right tube was swollen huge. They admitted me to the hospital and checked my level the next morning – it had dropped to 84. They sent me home and I continued to bleed and watch my numbers down to zero.

The right sided pain continued and they finally removed my tube 6 weeks later. It was sealed on both ends and full of old blood.

(9 months later I got pregnant again with my one tube left and my son is 13 weeks old)

I am not trying to scare you or discourage you, but the reality is that you are right to be cautious. I hope that you ovulated later than you thought and that your numbers will rise and everything will be fine. But I have had 2 TR babies and 2 m/c since my TR, so I know firsthand that it doesn’t always work out.

Please be aware of symptoms of an ectopic pregnancy and go to the doc ASAP if you have any.

Diane is so right… we have been on this journey together for months before out TR and everyday since. Your BFP is our BFP… I do have to say I am not as good a person as Diane lol, when I heard about your BFP I got so anxious to finally see one myself, but yes I am thrilled for you and DH. You both mean the world to us. The best news thus far is that you can get pregnant… and it gives us hope! Here’s to the 8-3-09 TR sisters and a happy and healthy nine months for you Beverly.

Well got my second hcg draw back this afternoon. Wasn’t good they went from 40 to 52 in 48 hrs. The dr is thinking ectopic. But I will go again friday for a 3rd draw to see if the hcg numbers will double.
I’m really sad but trying to stay hopeful! I have been praying for a baby for xmas for months! I really thought my prayers were answered and I was getting my baby! I guess god must have other plans for me!
