A vasectomy reversal is a surgical procedure that is performed when a man wants to reverse the effects of a vasectomy. There are two procedures that may be done to achieve this, the vasovasostomy and the vasoepididymostomy.

Vasovasostomy (VV):

A vasovasostomy is the standard procedure during vasectomy reversal surgery. During a vasovasostomy, the separated ends of the vas deferens are reconnected. Before the vas deferens are reconnected, the vas fluid is examined under a microscope and considered good if it has a clear non-opaque consistency and contains completely formed, healthy sperm. Having motile sperm is even better.

Vasoepididymostomy (VE):

When no vas fluid or low quality vas fluid is found during the vasectomy reversal procedure, a vas-to-epididymis (vasoepididymostomy) is necessary. If there is no sperm in fluid from the vas deferens coming from the testicle, it is concluded that there is a blockage upstream closer to the testicle. A blockage won’t allow sperm to pass through to the vas deferens, so connecting vas deferens to vas deferens and performing a vasovasostomy does not make sense in this case. A blockage in the epididymis upstream following vasectomy might have occurred due to inflammation, too much pressure in the epididymis, or scarring at the time of the vasectomy. During a vasoepididymostomy, the vas deferens are connected directly to the epididymal tubule in the epididymis to bypass the blockage.

Skill, experience and expertise are major factors that have a bearing on the success of vasectomy reversal. Other than that, ensure that your surgeon not only has had formal fellowship training but also is licensed to practise in your state or province.

Confirm that your micro surgeon /reversal specialist performs the reversal using an operating microscope, and 2-layer closure with 10-0 nylon sutures. It would also be wise to check out the surgeon’s statistics for number of reversals per year, number of successes, complication rates and whether or not he has a university appointment.

Reversing a Vasectomy can be done! But – it works best for men in good health who are in their forties or younger, and their partner is able to get pregnant. The results are even better if it has been less than 10 years since their original vasectomy and only a small part of the vas deferens was damaged by the previous surgery. A physical examination, tests, and review of the original vasectomy will need to be undergone to determine if the man is a possibility for this reverse vasectomy surgery. Only about 50% of the time will a man’s partner get pregnant after the vasectomy reversal surgery.

It is known that because sperm production continues, sperm debris accumulates in the system after a vasectomy, even though some of the debris is absorbed. The fluid left in the system thickens making it less likely to flow, the longer the interval between vasectomy and reversal. So, the sooner the reversal, the better the result.

Reverse Vasectomy is the most successful when a surgeon uses a microscope to do the surgery. Other factors include microsurgical precision and experience, delicateness and familiarity with the specialist equipment – the equipment and skill of a practised expert team is a major factor together with experience in being able to gently join the vas or epididymis. The reversal operation takes up to one hour each side and is not painful when the individual is awakened from a light general anesthetic. It can be done as an outpatient, but will require several days’ recovery at home. At least two full days quiet rest for initial healing, and refraining from intercourse for 3-4 weeks to allow trauma free healing. Ten weeks after the operation the first sperm count is taken and intercourse to obtain a pregnancy can recommence, as accumulated sperms made under pressure will have been passed.

This microsurgical reversal involves joining of the two ends of the vas (vasovasostomy) or, if epididymal obstruction has occurred, joining the vas to a section of the epididymis in which sperm are found (vasoepididymostomy). While the success of reversal can be gauged by the reappearance of sperm in the ejaculate, the only important marker of success is the occurrence of pregnancy in the years following surgery.

The probability of success is related to the years since vasectomy. In the US study, the pregnancy rate over the two years after surgery in men with less than three years between vasectomy and reversal were 76%, while the pregnancy rate was only 30% when the vasectomy had been undertaken eleven or more years previously. Bear in mind that the skill and experience of the surgeon certainly does have an effect on the success rate.

Because of the success rate of Vasectomy Reversals some surgeons recommend that a man make sperm bank deposits before having a vasectomy, just in case he changes his mind in the future about having more children.

The total cost of a Vasectomy Reversal falls somewhere between $4,000 to $20,000, all inclusive. In the United States, US$10,000 is the average cost.

A vasectomy is a surgical procedure that offers a permanent solution to birth control. It works by removing all or part of the vas deferens, the tubes that deliver sperm into a man’ s ejaculation, thus making the man infertile. In the U.S. alone, over 500,000 men opt to have a vasectomy each year. Of these men, it’ s estimated that five percent, for whatever reason, want the procedure reversed. For them, the best option is microsurgical vasectomy reversal.

But What is it and How is it Performed?

A vasectomy reversal acts to reconnect the two ends of the vas deferens that were severed in the initial procedure. A vasectomy reversal takes much longer than a vasectomy and is much more expensive. The reason for this is that the ends of the vas deferens are the size of a pinhead. The use of a microscope or other magnification device must be used in order to effectively complete the procedure. That’ s why microsurgical vasectomy reversal is so successful. During this procedure, a doctor or micro surgeon uses a microscope to enhance the view up to sixteen times. With such a good view of the surgical area, the surgeon is able to reconnect the two pieces of the vas deferens to allow the man to once more become fertile.

Microsurgical vasectomy reversal is a shorter, and often more effective, procedure than other reversal procedures. With microsurgical vasectomy reversal, the procedure generally takes a shorter time and is performed in an outpatient capacity. The patient is usually back to work in a week or two.

Microsurgical vasectomy reversal has proven to increase the chances for the procedure’ s success. That’ s why microsurgical vasectomy reversal is so popular. The success rate for the procedure is almost ninety-nine percent. Before microsurgical vasectomy reversal surgeons didn’ t use magnification tools. The success rate, as you can imagine, wasn’ t very high due to the difficulty of the surgeon being able to see what he was doing. Now, the use of a microscope makes microsurgical vasectomy reversal a very effective procedure for reversing the vasectomy process.

A man who has had a vasectomy and wants to reverse the process should seriously consider microsurgical vasectomy reversal. While very intricate, its success rate speaks for itself. It should be considered for any man wanting once more to birth children. Vasectomies weren’ t meant to be reversed, but microsurgical vasectomy reversal will definitely do the job right.

While a vasectomy procedure is considered permanent, it is medically possible to reverse vasectomies for some individuals. Because the success of a reversal is not guaranteed, an individual must feel sure that his childbearing years are complete when he makes the decision to undergo a vasectomy. During the procedure, a surgeon severs the vas deferens, the tube that delivers sperm into a male’s semen during intercourse.

A common question that patients ask prior to this operation is whether it can be reversed; sometimes it can.

A vasectomy reversal involves stitching the vas back together in a way that leaves the canal open.

There are several factors that contribute to the success rate of the vasectomy reversal procedure, including the following:

Experience of the surgeon: The surgeon rebuilds the vas channel by reconnecting the clipped ends of the vas deferens. The suture material (or surgical thread) that must be used to stitch the vas deferens back together is finer than a human hair–to see it adequately, a powerful microscope is used during the procedure. A surgeon accustomed to performing this procedure can increase the likelihood of success.

Condition of patient’s tissues: Reconnecting healthy tissue is more likely to be successful than attempting to reconnect damaged tissue. Based on clues gained during and after the procedure, or vasectomy side effects, an experienced surgeon can generally predict how healthy an individual’s tissues will be after a period of time. (Note that few patients experience vasectomy pain during the original procedure; also, discomfort alone is not an indication of tissue damage.)

Amount of time since vasectomy: The less time that has lapsed since the original vasectomy procedure, the greater chance a patient has for a successful reversal. Generally, males who undergo a vasectomy reversal less than 10 years following the original surgery can expect a greater likelihood of success. Those who attempt reversal three years after a vasectomy reportedly have a 97% chance of success. It must also be considered that over time, sperm quality diminishes; this means that even with a successful vasectomy reversal, in some cases, new sperm are unable to fertilize the egg.

The bright news for individuals who have undergone the procedure and vasectomy recovery only to experience life changes that cause them to reverse the procedure is that a reversal can be explored and may be successful. Individuals who have specific questions or concerns should speak with a vasectomy surgeon to confirm the likelihood of success considering their unique circumstances.

The Two Types of Vasectomy Reversal

There are two types of vasectomy reversal: vasovasostomy and vasoepididymostomy. Vasovasostomy is when the surgeon sews the cut ends of the vas deferens back together. This is the most common type of vasectomy reversal. Vasoepididymostomy is more complicated, but is performed when there is blockage in the epididymis. It is done by attaching the vas deferens to the epididymis, hence its name.

Length of Time

There is a correlation between the time your vasectomy was performed, and the time you want to have a vasectomy reversal. The longer you wait to have the vasectomy reversal done the more your chances decrease of being able to regain your fertility. Up to three years after your vasectomy your chances are higher to get your fertility back. After three years your chances begin decreasing slowly the longer you wait. While there is no period of time considered too long to perform a vasectomy reversal, the sooner you do so after your vasectomy the better the chances are that you will be able to father a child.

What Medical Breakthrough Makes Vasectomy Reversal Possible

The medical breakthrough which makes vasectomy reversal possible is the relatively new development of microsurgical techniques. Prior to the discovery of these techniques vasectomy reversal was thought to be too complicated, as well as unlikely to help men who had a vasectomy regain their fertility. Microsurgery or the use of an operating microscope during surgery allows the surgeon to see the vas deferens in much greater detail and as such to perform the vasectomy reversal with much greater success.

Success Rate

According to recent studies after vasovasostomy sperm is present in semen in about 85-97 percent of men. About fifty percent successfully father a child after the procedure. In the case of vasoepididymostomy sperm shows up in the semen in about sixty-five percent of the men. Only about twenty percent achieve fatherhood after this procedure.

Choosing a Surgeon

Choosing the surgeon to perform your vasectomy reversal is one of the most important tasks in this process. It is critical that you choose a surgeon who is well versed in microsurgical techniques if you want the procedure to be successful. It is also important to ask your surgeon of choice if he/she can do a vasoepididymostomy with an operating microscope. The surgeon also needs to have experience in checking out the quality of the vas fluid, figuring out the best location to do the vasoepididymostomy, as well as, assessing the signs of epididymal blockage.

These are the issues you should consider should you choose to have a vasectomy reversal. The procedure is not guaranteed to restore your fertility, but should you decide the have the procedure done by a reputable surgeon versed in microsurgical techniques, your chances are good.

Statistically, five to ten percent of the over 600,000 men who annually undergo a vasectomy, will choose to have theirs’ reversed at some time. Vasectomy is the process by which the tube carrying sperm from the testicles out the penis is blocked. Vasectomy reversal is a micro-surgical procedure (done using an operating microscope) that requires up to 4 hours of operating time, wherein the tubes are reconnected, allowing sperm to pass through once again.

Before the development and refinement of microsurgical techniques, the results of vasectomy reversal were relatively poor. However, in recent years, with the development of better techniques and a larger pool of skilled microsurgeons to perform the reversal, excellent results can be obtained with a high chance of obtaining pregnancy. Vasectomy reversal is generally carried out in hospital using a general anesthetic.

Success rates of reversal vary depending on the surgeon. The pregnancy rates after reversal on average are only about 50%. Costs for the surgery range from $5,000 to $20,000.

Complications- Though rare, there may arise some difficulties following vasectomy reversal. These include the oozing of blood from the corners of the scrotal incision, which usually stops within 12 hours following the procedure. Some patients may incur a large bruise that settles with time and rest. Also, there are chances of an infection that is quite painful, accompanied by the scrotum acquiring a red colour. Immediately contact your vasectomy reversal doctor in case of any of the above complications!

Choosing your Vasectomy reversal expert- The reversal surgery is specialized and technical and demands extensive training in microsurgery and an experience of conducting several reversals throughout a year.

Pregnancy after vasectomy Reversal- The statistics for pregnancy post vasectomy, as mentioned above, are quite encouraging! After vasectomy reversal a couple may attain pregnancy within a couple of months. The standard duration is at least a period of 12 months. Most pregnancies occur with in a period of two years after the reversal.

Armed with the above knowledge, it is now time to make your decision. It is best not to delay vasectomy reversal because even if you do not plan a child for the next one or two years, it must be borne in mind that post vasectomy reversal pregnancy takes an average time of twelve months. In addition, the more you delay, the more affected are your chances of attaining patency. Hope this information helps you make an informed decision about your and your family’s future!
